The Caregiver is a member of the home care team who works under the supervision of the Office Manager, and Administrator, performs many personal care services essential to meet the client’s needs. The Caregiver is responsible for observing clients and their response to care, and documenting/reporting these observations. Client Care Coordinator

Position Description:

The Client Care Coordinator works to enhance the effectiveness in achieving Sandcastle’s goals of the high-quality care and customer service while demonstrating compliance with all regulatory agency standards. The Client Care Coordinator works directly with the client, the client’s family, and the care team. Each clients’ care team consists of the Scheduling team, the caregivers, and any 3rd party directly involved with the care of the client.

Position Responsibilities:

  • Collaborates with the care team regarding a client’s plan of care.
  • Serve as an educator and resource for each client (and family) in managing their own care in order to ensure optimal quality of care.
  • Identifies, evaluates, and acts to resolve any potential barriers to delivery of care.
  • Arrange for additional care appropriately according to needs of client.
  • Follow up with scheduling daily to ensure accountability.
  • Ensure paperwork is compliant with state guidelines.
  • Include HR Coordinator in all caregiver issues.
  • Bring all compliance concerns to the office manager in a timely manner.

Scheduling Coordinator

Full Time

Position Description:

The scheduling coordinator is responsible for managing all staffing processes, including client scheduling and service, scheduling resolution. The scheduling coordinator will work as part of the Sandcastle Scheduling team in conjunction with Case Management.

Position Responsibilities:

  • Manages ongoing client staffing processes, including:
    • Schedules staff for both new and existing client’s needs.
    • Manages staff absences to ensure that each client’s needs are being fulfilled.
    • Matches Caregiver’s skill level to client’s needs.
    • Reports promptly all quality assurance concerns to case management.
    • Reports patterns and trends pertaining to caregiver attendance and behavior to HR coordinator.
  • Answers office phones in a timely manner.
  • Checks messages every thirty minutes.
  • Meets regularly with team members to ensure quality client care.

HR/Onboarding Coordinator

Position Description:

The Onboarding Coordinator supports the Human Resources Coordinator and assists with the day-to-day human resources operations.

Position Responsibilities:

  • Answers phones for the HR department.
  • Conducts Caregiver interviews, and completes the 24, 48, and 72-hour follow-ups.
  • Creates and Uploads Caregiver personnel file in the Caregiver company drive.
  • Maintains confidentially of Caregiver personnel file.
  • Inputs Caregiver data into Wellsky.
  • Inputs Caregiver Availability into Wellsky.
  • Obtains and Maintains all required Certifications/Documentation (AHCA, State and Federal.)
  • Conducts AHCA Level II Background checks on all Caregivers.
  • Conducts county criminal record checks upon hire and yearly thereafter.
  • Submits all New Hire/Orientation paperwork to Caregiver via DocuSign.
  • Distributes and keeps track all Sandcastle Homecare Caregiver Uniforms.
  • Completes all Caregiver reference checks.
  • Adheres to all programs promoting Retention, Associate Appreciation, and Quality Assurance calls.
  • Investigates and responds to all allegations of Caregiver misconduct.
  • Reports all active and deactivated Caregivers to AHCA.
  • Maintains communication with the Scheduling Team regarding to Caregivers work status (i.e., change in availability, requested days off, any changes that affects the Client’s schedule.)
  • Documents all communications with Staff, Caregivers, Clients and Family Members, etc. in Wellsky.
